Parçalı sagittal mandibular ilerletici (modular mandibular advancer/MMA) apareyinin dentofasiyal yapılar üzerine etkilerinin incelenmesi

Abstract

Bu çalışmanın amacı; bir elastik sabit fonksiyonel aparey olan Parçalı Sagittal Mandibular İlerletici’nin; mandibular gelişim geriliğine bağlı iskeletsel sınıf II maloklüzyona sahip, hızlı büyüme atılım dönemini geçmiş hastalarda meydana getirdiği dentofasiyal değişiklikleri sefalometrik olarak incelemektir. Bu amaçla aparey uygulaması öncesi yaş ortalaması 15 yıl 2 ay olan, iskeletsel sınıf II maloklüzyona sahip, hızlı büyüme atılım dönemini geçmiş, aparey tatbiki öncesi ve sonrası ağız içi-ağız dışı fotoğrafları ve lateral sefalogramları mevcut 23 birey çalışmaya dahil edildi. Lateral sefalometrik analizler sonucunda; apareyin maksilla gelişimini kısıtladığı ancak mandibular büyüme üzerine etkisi olmadığı görüldü. Aparey tatbiki sonucu vertikal boyutlarda değişiklik olmadığı belirlendi ve üst dudağın geri hareketi hariç yumuşak dokularda anlamlı değişiklik görülmedi. Keser ilişkileri incelendiğinde overjet miktarının azaldığı, overbite miktarında anlamlı değişiklik olmadığı kaydedildi. Üst kesici dişlerin retrüze olduğu ve geriye eğimlendiği, alt kesici dişlerin ise protrüze olduğu ve öne eğimlendiği görüldü. Üst molarlarda distalizasyon ve intrüzyon, alt molarlarda mezializasyon ve ekstrüzyon meydana gelirken okluzal düzlem eğiminin arttığı kaydedildi. Parçalı Sagittal Mandibular İlerleticinin bu etkileri değerlendirildiğinde iskeletsel sınıf II maloklüzyonların dentoalveolar düzeltimi için uygulanabilecek bir sabit fonsiyonel aparey çeşidi olduğu görüldü.

The aim of this study is to investigate the cephalometric changes in the dentofacial structures of people who have skeletal class II malocclusion associated with mandibular retrusion and treated with the Modular Mandibular Advancer Appliance, which is an elastic fixed functional appliance used after pubertal growth spurt stage. For this purpose; 23 individuals that were past their pubertal growth spurt and also had skeletal class II malocclusion were included in the study, with a mean age of 15 years and 2 months. These individuals had pre and post-MMA application intraoral and extraoral photographs and also lateral cephalograms available. As a result of lateral cephalometric analyses; it was seen that the appliance restricted the development of maxilla but did not effect mandibular growth. It was determined that there was no change in vertical facial dimensions and there was no significant change in soft tissues except backward movement of the upper lip. When incisor relations were examined, it was noted that the amount of overjet decreased and there was no significant change in the amount of overbite. It was seen that the upper incisor teeth were retruded and retroclined, the lower incisor teeth were protruded and inclined forward. It was noted that the occlusal plane inclination increased as a result of distalization and intrusion of the upper molars and mesialization and extrusion of the lower molars. When these effects of MMA were evaluated, it was seen that the MMA is a functional appliance that could be applied for dentoalveolar correction of skeletal class II malocclusions.
