Üç Boyutlu Sonlu Eleman Analiz (SEA) yöntemi kullanılarak 1, 2, 3, 4 parçalı
segmental Le Fort I osteotomileri sonrası çiğneme kuvvetlerinin etkisiyle kemik
segmentlerde ve titanyum fiksasyon sistemlerinde meydana gelen stres birikimleri
ölçülerek, bunlara bağlı kemik segmentlerin ve fiksasyon sistemlerinin instabilite
durumlarının karşılaştırmalı olarak incelenmesi. Maksiller gelişim geriliği bulunan bir
hastanın DICOM dataları alınarak maksiller kemiğin 3 boyutlu modellemesi MIMICS
programıyla yapıldı. Kontrol modeli olarak daha önce SARPE operasyonu ile 5 mm
maksiller genişletme yapılmış modelde geleneksel Le Fort I osteotomisiyle maksilla 5
mm ileri alındı. Deney grubuna 2, 3 ve 4 parçalı Le Fort I teknikleri uygulanarak
maksillaya 5 mm genişletme ve 5 mm ilerletme yapılıp zigomatikomaksiller
desteklerden ve apertura piriformis bölgesinden 2 adet 1,7 mm standart 5 delikli L
plaklar ve kanin-1. premolar dişlerin vestibülünden 4 delikli düz plaklar, 1,6 mm
çapında ve 5 mm uzunluğunda titanyum vidalar ile SolidWorks programında sabitlendi.
Molar bölgesinden 250 N ve kesici dişler bölgesinden 125 N olarak uygulanan
modellerin ANSYS programı ile SEA analizleri yapıldı. Pmax değeri kortikal kemikte
ön kuvvetlerde model 3’teki düz plağın 2. ve 4. vida bölgesinde, kanselöz kemikte 4.
modeldeki düz plağın 4. vida bölgesinde; Von Mises değeri ön kuvvetlerde model 3’te
düz plağın 2. vida bölgesinde, Pmin değeri kortikal kemikte ön kuvvetlerde model
3’deki düz plağın 4. Vida bölgesinde, kanselöz kemikte ön kuvvetlerde model 4’teki
düz plağın 4. vida bölgesinde, total deformasyon miktarı ön kuvvetlerde model 3 ve 4’te
maksiller alt ön segmentte ölçülmüştür. Model 3 ve 4’te, model 1 ve 2 ‘ye göre yüksek
stres biriktiği, model 1 ve 2’nin daha stabil olduğu, plaklardaki en yüksek stresin büküm
yerlerinde olduğu, düz plakların 4 delikli standart plak yerine güçlendirilmiş modifiye
plaklar olması gerektiği, daha fazla klinik çalışmaya ihtiyaç olduğu görülmüştür.
Three Dimensional Finite Element Analysis (FEA) of the bone segments and the
titanium fixation systems formed by 1, 2, 3, 4 piece segmental Le Fort I osteotomies for
a comparative study of the instabilities, based on the measurements of the accumulation
of stress on these bone segments and fixation systems due to the force of mastication.
DICOM data from a patient with retarded maxillary development has been taken and
the MIMICS software has been used for 3 dimensional modelling of the maxillary bone.
As the control model, the maxilla was advanced by 5 mm on a model with previous 5
mm maxillary widening through the SARPE operation. 2, 3 and 4 piece Le Fort I
technique was applied to the experiental group for 5 mm maxillary expansion and 5 mm
maxillary advancement, and titanium screws 1,6 mm in radius and 5mm in length were
used to fix two 1,7 mm standard 5 hole L plates from the zygomaticomaxillary buttress
and the pyriform aperture area and 4 hole flat plates from the vestubulum of the Canine1. Premolar teeth in the SolifWorks software. ANSYS software was used for FEA
analyses of models applied as 250 N from the molar area and 125 N from the incisor
area. The following has been measured: Pmax value on the 2nd and 4th screw area of
the flat plane in model 3 for frontal forces on cortical bone, the 4th screw area of the flat
plate in model 4 for the cancellous bone, Von Mises value for frontal forces in model 3
on the 2nd screw area of the flat plate. Pmin value on the 4th screw area of the flat plate
in model 3 for frontal forces on the cortical bone and 4th screw area of the flat plate in
model plate for cancellous bone. Total deformation amount on the lower frontal
maxillary area in model 3 and 4 for frontal forces. It has been observed that more stress
develops in models 3 and 4 compared to the models 1 and 2, that the highest stress on
the plates occurs on the bending points, that the flat plates should be modified
reinforced plates instead of the 4 hole standard plates and that more clinical studies are
needed.
