Giriş ve Amaç: Periferik sinir yaralanmalarının etyopatogenezi farklı olsada cerrahi işlem zamanı ana amaç, sinir onarımı yapılarak distaldeki motor ve ya duyu kaybını minimalize etmektir. Tedavi planı yaralanmanın şekline, anatomik bölgesine ve etyolojisine göre değişmektedir. Cerrahi olarak ilk tercih en az gerginlik olacak şekilde yaralanma alanındaki sinirlerin uç uca nörorafisinin yapılmasıdır. Ancak bu her zaman mümkün olmamaktadır. Periferik sinir defektlerin rekonstruksiyonunda günümüzde hala altın standart yöntem olarak otojen sinir grefti ile onarım olsada donör alan parestezi, nöroma oluşumu, ağrı ve ek yara izleri oluşturma gibi komorbiteler, alıcı sinir ile greft sinir uçları arasındaki çap uyumsuzlukları ve uzamış ameliyat süresi otojen sinir grefti kullanımının dezavantajlarıdır. Bu gibi istenmeyen sonuçlardan dolayı periferik sinir defektlerinin rekonstruksiyonu için morbiditesi daha az, daha basit ve daha iyi fonksiyonel sonuçlar elde edilecek yöntemler ile ilgili araştırmalar yapılmaktadır. Materyal ve Metod: Bu araştırma Kocaeli Üniversitesi Deneysel Tıp Araştırma ve Uygulama Biriminde (KOÜ-DETAB) sıçanların sol siyatik siniri üzerinde gerçekleştirildi. Sinir rekonstruksiyonu çalışma grupları: Grup 1: otogreft ile sinir onarımı, Grup 2: nörotüp ile sinir onarımı ve Grup 3: siyatik sinir proksimal ucunun femoral arter distal ucuna koapte edildiği gruplar olarak planlandı ve tüm cerrahi operasyonlar aynı cerrah tarafından yapıldı. Deney hayvanları 8 haftalık sinir iyileşmesi beklendikten sonra fiziyoloji laboratuvarında yürüme testi yapıldı. İkinci cerrahi basamakta, önce deneklere EMNG yapılması sonra deneklerden histopatolojik inceleme için örnek alınması ve sakrifiye edilmesi şeklinde planlandı. Bulgular: Fonksiyonal olarak yürüme testi SFİ sonuçları istatistiksel olarak deyerlendirildiğinde Grup 1 ile Grup 3 arasında anlamlı fark olduğu (p<0,05), Grup 2 ile grup 3 arasında anlamlı bir fark olmadığı görüldü. EMNG yapılarak gruplar arasında Latans süreleri ve CMAP sürelerinin karşılıklı değerlendirilmesi zamanı anlamlı bir istatistiksel farkın olmadığı görüldü. Histopatolojik değerlendirme zamanı her 3 gruptada aksonal rejenerasyonun olduğu izlendi. Aksonal rejenerasyon en fazla otogreft ile onarım yapılan grupta görüldü. Grup 2 ile Grup 3 arasında aksonal rejenerasyon miktarı açısından anlamlı bir fark olmadığı tespit edildi. Sonuç: Yaptığımız araştırma sonucunda, arter sinir adaptasyonu ile sinir rekonstruksiyonu yaptığımız grupta fonksiyonal ve histopatolojik olarak sinir rejenerasyonunun geliştiği görüldü. Bununla da periferik sinir defektlerinin rekonstruksiyonunda ''İPEKYOLU TEKNİĞİ''-ninde bir alternatif ola bileceğini göstermiş olduk.
Introduction and aim: Although the etiopathogenesis of peripheral nerve injuries are different, the main objective during the surgical procedure is to minimize motor nerve damage or sensory loss in the distal by performing nerve repair. Treatment plan varies according to the type of injury, anatomical region and aetiology. The first choice of surgery is to perform end-to-end neurorrhaphy of the nerves with minimal tension at the site of injury. However, this is not always possible. Although the gold standard method of repair for the reconstruction of peripheral nerve defects is still with autogenous nerve graft, donor area paresthesia, neuroma formation, pain and comorbidities such as creating additional scars, diameter mismatch between recipient nerve and graft nerve endings and prolonged operation are the disadvantages of autogenous nerve graft use. Due to such undesirable consequences, studies are being conducted on methods for reconstruction of peripheral nerve defects with less morbidity, simpler and better functional results. Material and method: This study was performed on the left sciatic nerve of rats in Kocaeli University Experimental Medicine Research and Application Unit (KOÜ-DETAB). Nerve reconstruction study groups that were planned: Group 1: nerve repair with autograft, Group 2: nerve repair with neurotube, and Group 3: where the proximal end of the sciatic nerve was coapted to the distal end of the femoral artery, all surgical operations were performed by the same surgeon. After 8 weeks of nerve recovery, the animals were tested for gait in the physiology laboratory. In the second surgical step the plan was to perform EMNG on the subjects followed by sampling and sacrifice for histopathological examination. Findings: Functional gait test SFI results were evaluated statistically, there was a significant difference between Group 1 and Group 3 (p <0.05), but there was no significant difference between Group 2 and Group 3. When latency and CMAP times were compared between groups by performing EMNG there was no significant statistical difference. Histopathological evaluation showed axonal degeneration in all 3 groups. Axonal regeneration was mostly seen in the repair group with autograft. There was no significant difference in the amount of axonal regeneration between Group 2 and Group 3. Results: As a result of the study, it was seen that nerve regeneration developed functionally and histopathologically in the group where the nerve reconstruction was performed with artery-nerve adaptation. With this, it has been shown that it may be an alternative to ''SILKWAY TECHNIQUE'' in the reconstruction of peripheral nerve defects.
