Özet: H diffüz guatr, hipertiroidi, bazı hastalarda görülen oftalmopati ve dermopati ile karakterize otoimmun bir hastalıktır. GH’nın patogenezinden Tiroid Stimülan Hormon Reseptörüne (TSHR) karşı gelişen antikorların rol aldığı otoimmun mekanizmalar sorumlu tutulmakta; TMNG’nin ise, zaman içinde otonomi kazanarak toksik evreye geçen ötiroid multinodüler guatrdan kaynaklandığına inanılmaktadır. Günümüzde mekanizmaları tam anlaşılmamış olmakla birlikte, etyoloji ve patogenezlerinin farklı olduğu bilinen GH ve TMNG’nin, hastaların tiroid dokusundafarklı protein ekspresyonlarına yol açacakları düşünülmüştür. Çalışmamızda, GH ve TMNG hastalarının tiroid doku örneklerinin protein profillerini proteomiks yöntemler kullanarak karşılaştırmak ve farklılık gösteren proteinlerin hastalıkların patogenezindeki rolünü araştırmak amaçlanmıştır.
Çalışmaya, Kocaeli Üniversitesi Tıp Fakültesi İç Hastalıkları Anabilim Dalı, Endokrinoloji ve Metabolizma Hastalıkları Bilim Dalı polikliniğinde Graves hastalığı tanısı ile takip edilmekte olan 12 hasta (n=12, grup G) ve kontrol grubu olarak TMNG tanısı ile takip edilen 12 hasta (n=12, grup T) alınmıştır. Çalışmaya dahil edilen Graves hastalarının yaş ortalaması 36 ± 9,9; TMNG hastalarının yaş ortalaması 54 ± 12,7 yıl olarak bulunmuştur. Hastaların tiroid dokusu örneklerinden elde edilen özütlerin protein içeriği iki boyutlu jel elektroforez (2D-JE) teknikleri kullanılarak ayrıştırıldıktan sonra, proteinler MALDİ-TOF kütle spektrometrisi ile tanımlanmıştır. MASCOT yazılımı kullanılarak isimlendirilen proteinlerin fonksiyonları çeşitli veri bankalarının internet erişimleri kullanılarak belirlenmiştir.
Çalışmamızın sonunda GH grubunda 15 adet protein spotunun TMNG grubuna göre faklı eksprese olduğu görülmüştür. Bu proteinlerin hücre içerisinde iki metabolik yolakta rol aldıkları saptanmıştır. Birincisi hücre büyümesi-farklılaşması ile ilişkili olan yolaklar [Prelaminin A/C (LMNA), ‘’Makrophage Capping Protein’’ (CAPG), alfa enolaz (ENO1), ‘’Rho GDP-dissociation inhibitor 1’’ (ARHGDIA), Peroxiredoxin 2 (PRDX2), Catepsin B (CTSB), Catepsin D (CTSD), Apolipoprotein A1 (APOA1), ‘’Heat shock Protein β1 (HSP β1) proteinleri]ve diğeri enerji metabolizması ile ilişkili [Selenyum bağlayıcı protein 1 (SELENBP1), Kreatin kinaz B (CKB), Suksinil-KoA:3-ketoasid Koenzyme A transferaz 1(OXCT1), Dihidrolipoil
dehidrogenaz (DLD), Karbonik Anhidraz (CA1), Triozfosfat izomeraz (TPI1) proteinleri]yolaklardır.
Çalışmamızın sonucunda, hücre proliferasyon yolakları ile ilişkili olan proteinlerin bir kısmıçeşitli tümörlerin proteomiks çalışmalarında da saptanmıştır. Bu çalışmamızın önemli bir sonucudur. GH’da tirotoksikozunmetabolizma artışı ve hücre proliferasyonuna yol açtığı, bu sırada kullanılan moleküler mekanizmaların malign hastalıklardaki hücre proliferasyonu sırasında da kullanıldığı düşünülebilir ancak, toksik multinoduler guatrda da tirotoksikoz olmasına rağmen bu proteinler eksprese edilememiştir. Diğer yandan sonuçlarımız çeşitli veri tabanlarında analiz edildiğinde eksprese edilen proteinlerin pankreas tümörü, hepatoselüler karsinom ve kolorektal kanserler gibi tümörlerle ilişkili olduğunu gösterilmiştir. Çalışmamızdan elde ettiğimiz bir diğer önemli sonuç ise, eksprese edilen proteinlerinpankreatit, tip 1DM, Tip 2 DM, insulin direnci, metabolik sendrom ve MODY gibi glukoz metabolizması ile ilgili hastalıklarla ilişkilendirilmesidir. GH’na sıklıkla Tip 1 DM’un eşlik ettiği bilinmekle beraber, çalışmamızın sonuçları GH ile birlikte görülen diyabetin patogenezinde otoimmünite dışında diğer nedenlerin de söz konusu olabileceğini düşündürmektedir.
Graves Hastalığı ile ilgili proteomiks çalışmalarının az sayıda olması ve günümüze kadar yapılmış çalışmaların daha çok tiroid malignitelerini araştırması nedeniyle literatürden elde edilen bilgi sınırlıdır. Sonuçlarımız, daha geniş çalışmalarla desteklenmeyi gerektirmektedir ve GH’nın patogenezini anlamaya yönelik gelecekte yapılacak araştırmalar açısından umut vericidir.
Summary: Graves’Disease is an autoimmune disease characterized by diffuse goiter and hyperthyroidism even though it shows symptoms of ophthalmopathy and dermopathy in some patients. Autoimmune mechanisms which contain antibodies developed against Thyroid Stimulating Hormone Receptor (TSHR) are held responsible for thepathogenesis of GD and TMNG is believed to be caused by euthyroid multinodular goiter gaining autonomy in time and going into toxic stage. With their mechanisms still not fully comprehended today, GD and TMNG are known to have different etiologies and pathogenesises and they are considered to cause different protein expressions in thyroid tissues of the patients. This study compares the protein profiles of GD and TMNG patients’ thyroid tissue samples using proteomics methods and investigates the role of the differentiating proteins in the pathogenesis of the diseases.
12 patients (n=12, group G) diagnosed with Graves’ disease and followed in Kocaeli University Faculty of Medicine Internal Diseases Department, Endocrinology and Metabolic Diseases Polyclinic and 12 patients (n:12, group T) diagnosed with TMNG and followed as the control group are incorporated into the study. The average age of the Graves’ disease patients is 36 ± 9.9 and the average age of the TMNG patients is 54 ± 12.7. The protein content of extracts obtained from the patients’ thyroid tissue samples was separated with two-dimensional gel electrophoresis (2D-GE) method and the proteins were identified via MALDI TOF mass spectrometer. The proteins were named by MASCOT software and their functions were determined by using the internet connection of various data banks.
At the end of the study, 15 protein spots in GD group were observed in different expression than those of TMNG’s. These proteins were detected to play role in two different metabolic pathways in cell. The first one is cellular growth-differentiation related pathways [Prelaminin A/C (LMNA), ‘’Makrophage Capping Protein’’ (CAPG), alfa enolaz (ENO1), ‘’Rho GDP-dissociation inhibitor 1’’ (ARHGDIA), Peroxiredoxin 2 (PRDX2), Catepsin B (CTSB), Catepsin D (CTSD), Apolipoprotein A1 (APOA1), ‘’Heat shock Protein β1 (HSP β1) proteins] and the other is energy metabolism related [Selenium-binding protein 1 (SELENBP1), Creatine kinase B (CKB), Suksinil-KoA:3-ketoasid Koenzyme A transferaz 1(OXCT1), Dihidrolipoil
dehidrogenaz (DLD), Carbonic Anhydrase (CA1), Triosephosphate Isomerase (TPI1) proteins] pathways.
As a result of the study, some of the proteins related to cell proliferation pathways were detected in the proteomics studies of various tumors. This is a significant result of the study.It could be considered that thyrotoxicosis in GD causes metabolism increase and cell proliferation and the molecular mechanisms used in the meantime are also used during the proliferation of malign diseases. However, these proteins could not be expressed even though toxic multinodular goitre, too, has thyrotoxicosis.On the other hand, when the results were analyzed in various databases the expressed proteins were observed to be related to pancreas tumor and tumors such as hepatocellular carcinoma and colorectal cancer. The other significant result of the study is that the expressed proteins are correlated to pancreatitis, T1DM, T2DM, insulin resistance, metabolic syndrome and diseases related to glucose metabolism such as MODY. While it is known that GD is usually accompanied with DM Type 1, the results of the study suggest that there could be other causes for the pathogenesis of diabetes seen with GD other than autoimmunity.
The information in literature is limited since there are few proteomics studies about Graves’s disease and the studies up to now have rather researched thyroid malignancies. The results need to be backed up by more comprehensive studies and the results are promising in terms of future studies as to understanding the pathogenesis of GD.
