Özet: Hepatit B Virusu (HBV), yüksek replikasyon kapasitesinin olması ve ters transkripsiyon işleminde hata düzeltme yeteneğinin olmaması nedeniyle, yüksek mutasyon sıklığına sahip bir virustur. Tedavi öncesinde NA direnci ile ilişkili mutasyonlar meydana gelebileceği gibi, kronik hepatit B (KHB)’nin nükleoz(t)id analogları (NA) ile uzun süreli tedavisinde de antiviral ajana dirençten sorumlu mutasyonlar gelişebilmektedir. Günümüzde KHB infeksiyonunda antiviral tedaviye başlama kararı verilmesi, tedaviye yanıtın izlenmesi ve antiviral direnç saptamada pahalı ve deneyim gerektiren bir yöntem olan HBV DNA düzey tayini ile yapılmaktadır. Bu çalışmada KHB tedavisinde kullanılan oral antivirallerin pol geni ve S geni üzerinde yaptığı mutasyonların araştırılması ve bu mutasyonların klinik ve epidemiyolojik öneminin saptanması, uzun dönemde ilaç direnci gelişiminin ve bunun yol açacağı sonuçların belirlenmesi amaçlanmıştır. İkincil olarak, kantitatif HBsAg (qHBsAg) titrelerinin ilaç direncini belirlemede erken bir marker olup olmadığının saptanması amaçlanmıştır. Çalışma Kocaeli Üniversitesi Araştırma ve Uygulama Hastanesi’nde 2009 ile 2014 yılları arasında yapıldı. Çalışmaya KHB infeksiyonu olan naif 55 hasta ile 39 inaktif HBsAg taşıyıcısı olmak üzere toplam 94 hasta dahil edildi. Hastaların qHBsAg düzeyleri ile HBsAg (S/Co) ve HBV DNA düzeyleri takip edilerek korelasyon analizleri yapıldı. Tedavi öncesinde ve tedavi sırasında breakthrough gelişen hastalarda HBV DNA sekanslama analizi yapıldı. Çalışmamızda HBsAg düzeyleri ile virolojik ve histopatolojik bulguların ilişkisi incelendiğinde ileri fibrozu olan HBeAg pozitif hastalarda qHBsAg seviyelerinin daha düşük olduğu gözlendi. Tedavi alan hastaların tamamı ele alınarak yapılan korelasyon analizlerinde başlangıç qHBsAg ve HBV DNA seviyeleri arasında istatistiksel olarak anlamlı orta düzeyde bir korelasyon olduğu görüldü. Hastalar fibroz durumuna göre gruplandırıldığında ise başlangıç qHBsAg ile HBV DNA arasında düşük fibrozu olan hastalarda yüksek düzeyde bir ilişki varken, yüksek fibrozu olan hastalarda ise orta düzeyde bir ilişki olduğı görüldü. Çalışmamızda peg-IFN tedavisi ile qHBsAg seviyelerinde ki azalmanın daha hızlı olduğu ve daha yüksek oranda tedaviye yanıt alındığı görüldü. İnaktif HBsAg taşıyıcıları ile HBeAg negatif KHB hastalarını ayırmada qHBsAg düzeylerinin tanı koymadaki etkinliğinin iyi derecede olduğu sonucuna varıldı, cut off değeri ise 2188 IU/mL olarak belirlendi. HBsAg (S/Co) cinsinde olan değerlerinin ise tedaviyi izlemde kullanılamayacağı sonucuna varıldı. Yapılan analizlerde iki hastada naif dönemde primer ilaç direnci mutasyonu, iki hastada doğal gelişen ADAPVEM paterni saptandı. Çalışmamızda bu mutasyonların NA tedavisine bağlı oluşabileceği gibi HBV’nin doğal kinetiği nedeniyle naif hastalarda da oluşabildiği sonucuna varıldı.
Abstract: The Hepatitis B Virus (HBV) is a virus having high mutation frequency due to having a high replication capacity and not having error correction capability in reverse transcription. Mutations related to NA resistance can occur prior to the treatment and mutations responsible for the resistance to antiviral agents can develop in the long-term treatment of chronic hepatitis B (CHB) with nucleos(t)ide analogs (NA). Nowadays, the decision of starting antiviral treatment at CHB infections is given by HBV DNA level assay which is an expensive method requiring expertise on monitoring the response to the treatment and determining the antiviral resistance. In this study, it is aimed to examine the mutations created by the oral antivirals used in CHB treatment on pol gene and S gene, to determine the clinical and epidemiological significance of these mutations, and to specify the development of drug resistance in long terms and the results it may cause. Secondly, it is aimed to determine whether the quantitative HBsAg (qHBsAg) titers are early markers for detecting drug resistance or not. This study was carried out in Kocaeli University Medical Faculty Hospital between the years 2009 and 2014. 55 naive patients with CHB infection and 39 inactive HBsAg carriers, totally 94 patients, were included in the Study. Correlation analyses were performed with monitoring qHBsAg levels and HBsAg (S/Co) and HBV DNA levels of the patients. HBV DNA sequencing analysis was done at the patients developing breakthrough pre and during the treatment. During our study, it was monitored that at HBeAg positive patients having severe fibrosis qHBsAg levels were lower when the relations of HBsAg levels and virological and histopathological findings were analyzed. It was seen that in the correlation analysis done by including all patients receiving the treatment, a statistically significant middle level correlation existed between the initial qHBsAg and HBV DNA levels. When the patients are grouped according to the state of fibrosis, it was seen that among the patients with low fibrosis score there was a relation at high level between the initial qHBsAg and HBV DNA, and there was a middle level relationamong the patients with severe fibrosis. In the study, it was noted that the decrease in qHBsAg levels was faster with peg-IFN treatment and the response to the medical treatment was at a higher rate. It was concluded that the efficiency of qHBsAg levels on making diagnosis is at a good degree to distinguish inactive HBsAg carriers and HBeAg negative CHB patients, and the cutoff value was determined as 2188 IU/mL. It was concluded that values in terms of HBsAg (S/Co) cannot be used for monitoring the treatment. In the analyses made, primer drug resistance mutation during the naive period was determined in two patients and naturally developed ADAPVEM pattern was determined in two patients. It was concluded in the study that these mutations can develop in connection with NA treatment and that they can develop in naive patients due to the natural kinetics of the HBV.
